"Clanging" puts the fear in me. TOBs can definitely rattle or grind, but they are not big, heavy, or metal enough to clang. You'd need to really clarify that sound. What you describe sounds like a substantial transmission problem. I guess the upside is that the labor for TOB and transmission replacement is pretty much the same, so all you're out is the cost of a replacement transmission. They aren't horrifically expensive and I wouldn't inherently let that sort of a problem scare me off from a particular car.
Regardless of what the actual problem is you don't get to charge a premium price for a car that needs work of any type. IMHO, the transmission is going to need to come out no matter what, and the price needs to reflect that *at least*. If you like the car, maybe agree to buy the car for the premium price once the problem is resolved or get a professional opinion (dealer, etc.) and then negotiate based on the feedback.
