Here at work we get in real deep $&!# if we ever leave a tank without the cap, and it's not strapped down really good. Do people really carry those tanks around with a regulator mounted? I'm not a diver, but I'm even kinda dissapointed to learn that scuba tanks don't(?) have caps to cover the shutoff valves.
With threaded connections (stress concentrations in the root of the threads), it's probably easier than you think to break off the valve. We've done tests (in a blast cell) about rupturing small bottles the size of a thermos. Their destructive power is amazing.
