Explain that these problems (other than the starting issue) are things that you have experienced before and can fix yourself. Add that you have a trustworthy indeipendent mechanic who you will ask to investigate the starting problem, and that the other issues will undoubtedly amount to considerable expense. Costs that you don't feel they should pay, and that you would rather hire a less expensive mechanic to fix the other one.
They probably feel they are responsible for these things going wrong (the gear shift issue they are, but you have fixed it before yourself), and want to make things right.
Acknowledge it is a higher mileage car that, although was fine when you lent it to them, is prone to sudden bouts of minor annoyance issues without warning.
Hopefully, neither of you will get hit with a big repair bill.
