Living and driving in the metro-NYC area, staying close to the person ahead of you is almost necessary, because people will use any opportunity to change lanes in front of you if they can. Especially when traffic is backed up.
So to answer you questions... I follow close enough to prevent someone from cutting in front of me (in traffic), but far enough away to give me time to react. At higher speeds I usually keep about 1 - 1 1/2 car lengths between me and the person ahead of me. Unless the person is going dangerously slow and I don't have time to slow down enough to their speed, I typically do not tailgate excessively.
I try not to do "offensive driving" maneuvers like speeding up to "block" someone, because doing stuff like that is just as unpredictable and dangerous as the weavers themselves are. But I will keep my distance close consistently so as to prevent weaving.
