Don't send it out with resumes. It will either be ignored and may not get to the person who really makes the hiring decision.
A line at the end of your resume "References available upon request" is standard, and you should have copies of the letter ready to send when references are requested. You could also have a copy with you when you go on an interview.
Be sure your boss is also OK with being called as a reference--surprising, but some who will write a letter don't want to be called (concern about certain liability issues).
Also, some people make it a practice to call their reference to give them a heads up when they have given the reference's name to a company that is considering them. Nice courtesy.
