If it truly does have frame damage you might push for it being totalled. That will come as a result of cost to fix as new -- on serious frame damage that can't be fixed as new it will be totalled. The insurance appraiser probably won't total it on the spot if it is questionable -- he will wait until the body shop begins to look for more money to fix or deems it unrepairable.
What I would do is find an indpendent appraiser (outside the shop and insurance appraiser) to do a review of the vehicle. You will need to do this anyway to make an inherent diminshed value claim after the repairs.
Using this guy upfront shouldn't cost you any more than using him after the fix and he can participate throughout the process and critique the work of the body shop. Shop around for an appraiser you like - A while back I paid $275 for one on my wife's old car, but I got quotes for $275 to $500. The insurance company generally will not reimburse this cost, but you should make it up in the diminished value claim if the guy is good.
All this said ask your insurance company about the process to make sure this is money well spent. I was working with Allstate and this was what worked for them. Some will pay for an outside appraiser and some will not. Some may automatically provide a diminished value amount (if you ask) and some make you prove it.
Crappy process. I feel for you. This was a while ago for me and it still brings back headaches.
