CarFax markets themselves as if they get every record in existence for every car but they don't -- that fact is in the fine print. They almost always get DMV records (titles and emissions generally), and maybe some service records, but the police reports for minor accidents, etc. and few and far in between. It's because they have to go out to the owner of every data feed and negotiate updates, and that's just not available or done for every place in the country yet.
Their 'outstanding recall' info has never been accurate for four cars I ran through it (that feed is from the manufacturers).
If is fairly rare to have it miss a salvage title brand, although it may take much time for it to reflect. It's possible in that state that the payoff/re-purchase might not trip the logic to mark it as an update (at the DMV that is -- it was your car, it's still your car, who knows how they 'file' that, how that differs from the neighboring state, etc.) These differences do raise doubt, so...
That's why a bad carfax is worth something but a good one means 'proceed with caution.' Even though they do disclaim this extensively in the fine print, I don't think they make too good an effort to educate the users about how 'good' a 'good' report may be.
