Bad Driveway Crack - how to repair?

This sucker is about 3.4 inch wide and 1.5 inches deep and seems to be getting worse every year. I tried last year filling with sand and sealing the top with driveway caulk but that did not even make it through the winter (you can see in the picture some pieces left from that effort). This goes across about 10 feet of the driveway and there is another that is almost as big. What is a good permanent repair? Driveway is otherwise in fine shape so I don't want to repave.
