I think your analysis is spot on. It's a real bear to clean the interior lens elements--very labor-intensive. The mildew eats into the anti-reflective coating and even into the glass in some cases. It's generally only worth it for expensive and collectable lenses, and not many shops tackle it.
Spot-cleaning the exterior mildew is worth a shot, and I'd try shooting with them to see whether image quality is affected much. If there are just a few you probably can't tell, otherwise you can use them for soft-focus portrait lenses ;-)
