As JP said, call Verizon and have them disable and log the ESN (electronic serial number) as stolen so it can't be activated again.
As far as a replacement phone is concerned, all you need to do it buy another - just be prepared to pay dearly. When you activate a phone, the price is subsidized by Verizon to get you to join. So the $50 RAZR when you activated service is probably around $300 to outright buy. You can get them on Ebay but be cautious - who knows what you're really buying. You can also buy them directly from Motorola or Verizon dealers online. Once you get the new one, call Verizon w/ the new ESN and they can activate it for you.
Don't worry about having insurance - in my experience its more hassle than it's worth. They'll give you a replacement phone but it's a refurbished phone and I guarantee it will have issues. Save yourself time and hassle and buy a new one.
