My doctor gives me a receipt with the description and the code. When I got some metal in my eye, the ophthalmologist (eye doctor and not an optometrist, who also claim to be eye doctors but go to optometry school, not medical school) used the code:
65222 Removal of foreign body from cornea
Looking at the receipt, I know some CPT codes (procedures, like oil change if the body were a car) and ICD-9 codes (diagnosis, like flat tire if the human body were a car).
