Another place for mass-market stuff is Rio Grande (http://www.riogrande.com), a wholesaler to the jewelry trade. You'll need to register to get on the site and order. They sell everything from raw materials to assembled jewelry, and everything a jeweler needs, including jewelry boxes for resale. Decent stuff, but China mass-produced.
I have to second Bill's comments - nothing beats an artisan. I highly recommend the higher-end Crafts shows - this ain't no popsicle art. You will find plenty of woodworkers selling some incredibly innovative jewelry boxes at a variety of prices. And most will take commissions if you want something bigger/smaller/fancier/simpler/cheaper. We're coming into the season. You want to look for juried shows - vendors don't get in unless their work is up to snuff.
Check out http://www.paradisecityarts.com, as they hold several shows around the East throughout the year. Also Google Arts and Crafts Shows. The League of New Hampshire Craftsmen hold a 10 day one up in, well, New Hampshire that is incredible.
Also check around for Crafts centers - schools that teach pottery, jewelry making, etc. An example is http://www.guilfordartcenter.org, to name one of many, many, many. Most have galleries to shop at, and again, if you don't see what you want, they can put you in touch with the artists.
You can also Google "Woodworker" "Jewelry boxes" and you'll get a lot of links of artisans (and a lot of other dreck, as usual.) You might find a few local craftsmen and women.
I'm a big believer in supporting your local artists.
