have been reported with their cars that appear to be unrelated to an accelerator pedal being physically stuck on either carpeting or floor mats. In some cases, drivers who claim to have attempted to pry the accelerator up with their shoe were unable to do so, as though something else was forcing the vehicle to go wide open throttle. WOT would also explain why they lost all braking ability after pumping the brake pedal one or two times.
I expect some of these problems are either electrical or software-related. Supposedly some Toyota cruise control systems will allow WOT to quickly "resume" a previous speed. Perhaps an electrical or software malfunction causes the vehicle's computer to think the driver wants to resume cruise control at some unintended high speed?
