Engineers can design a car specifically for the crash tests, but real world crashes rarely come close to imitating a crash test. The obvious difference is a small car vs. a large car. The small car may do well going into a barrier, but when hit by a large, heavy car the impact forces are entirely different. And in real world, cars slam into each other at different angles, different speeds, etc. Crash tests are artificial, but designed to minimize variables so cars can be compared on an equal basis, but cars rarely crash on an equal basis.
So we have the IIHS numbers. IIHS stands for Insurance Institute for Highway Safety, an insurance industry organization and the numbers reflect what the industry has paid out for injury claims. I suspect they use the numbers in rating policies, the process of calculating what you will pay.
One problem with the IIHS numbers is that one has to assume what insurance companies pay out in claims for injuries has some relationship to injury severity.
We don't have a good metric for car safety, but for my buck, I think the IIHS ratings are the best we have.
