When I checked the connection, I hadn't inserted one of the wires deep enough into the blade style connector so it was basically end of the wire contact against the blade. A millimeter either way would have either worked all the time or not at all.
The car delivers 11.4 V in the accessory position vs. 13.8 V when running (or maybe it was 11.8 and 13.4?). That difference was enough!
