...I heartily recommend Apple's calendar creator built into the desktop version of iPhoto.

I've done calendars for family the past two years, and plan to do another one real soon. The main things I like are the size: 10.5" x 13" page size, which becomes 21" x 13" when folded open and hung. They come in a nice sleeve, and are printed on a heavy card stock, not a lighter weight paper.
You do all the setup, adjustments, and ordering of the calendar in iPhoto. You can add many photos, and even add photos to specific dates, as well as adding birthdays, anniversaries, etc. to any date you wish. You can also use a different photo on the front and back covers, add text below or over your photos.
I keep going back because of the size and quality of the calendars. Most others I've seen print on 8.5" x 11" paper, usually not a heavy stock, and don't charge substantially less. Calendars through Apple run about $20 for a 12-month calendar. Extra months cost extra (of course), but, AFAIK, more photos don't cost anything extra.
