If you're talking about the "One" I think it uses it's altimeter to sense elevation changes.
From Fitbit web-site:
Sensors and Components
3-axis accelerometer
Altimeter
Vibration motor
I have a "One" and it really is just a glorified pedometer. For me the value-add is in the app the helps me estimate/track total caloric intake and output. I know, somewhere out there in the Internets all this information about my daily caloric intake is being logged and calculated. Really pretty boring information I think?
