College for Creative Studies (CCS) in Detroit along the the Art Center in LA produces about 75% of auto designers working at design studios in the US. To get in these schools, you need a good portfolio. If you're in high school, talk to your art teachers about putting one together. They can help you tremendously with that. That's if you want to get into the artsy side of designing. You don't need to be an engineer to be one of these, but fundamental knowledge of cars, proportions, etc. is needed.
If you want to be in the more technical area of design, then yes, you do need to be an engineer. Here the possibilities are endless. Do you you want to design engines, transmissions, etc., then go for a mechanical degree. Do you want to draw sheet metal in the computer? Some schools offer auto body design and many CAD classes. But from a creativity standpoint, you are very limited here. You are basically turning the designs studios sketches and computer renderings into manufacturable, and engineered designs in a CAD system which you need to be proficient in.
You're not responsible of designing from scratch the next Saab.
Also, you may be designing underbody structures to withstand crashes. Here you may want to go for either a mechanical engineering degree or structural engineering degree. As you can see, "design" means many things in the automotive sense.
