The vast majority of cars in Europe have manual transmissions. Only the very high end ones offer automatics, and not many of those are sold.
The vast majority of cars in America have automatic transmissions. So the brakes have to be bigger, the engine bigger to provide equivalent thrust, and the cars are just big and heavy.
50% of the vehicles sold in America are now trucks, so people want big cars to compete with all those big trucks. Even the lowly Honda Civic is now a pretty big car compared to what it started out as.
So if Ford brought some of its small really cool cars (I agree that the KA! is pretty sweet looking), they'd sell a few, but it wouldn't really be worth the effort.
Even SAAB has to put big displacement V-6's into its 9-5 to get Americans to buy it. In Europe, the 9-5 comes with either a 2.0 or 2.3L 4 cylinder, with the 3.0 "offered". Due to the taxes on displacement, that car is very, very expensive.
