Good because they (usually) pay for things people wouldn't get themselves, like preventative care and an adequately detailed evaluation for problems they want evaluated. When patients are paying out of pocket, in my experience, lots of time is spent negotiating about what is the minimum amount of testing that can be done. If the insurance company is paying, the discussion focuses more on the risks of the test to the patient and what you do with a positive result.

Bad because they hold everybody by a thread. Denials of coverage occasionally lead to very bad outcomes and are, at the very least, a major headache. Also bad because, in the U.S., such a high percentage of our health insurance premiums go to overhead. I can't believe that the average family racks up the kind of health care bills that are reflected in these ridiculous premiums.
