1. Yes, sort of. I believe on '94-ish 2.3l non-turbo cars, both 900s and 9000s. They had a standalone DI system, coupled with conventional fuel injection.
2. Yes.
3. I don't see any inherent limitation on using DI or DI/APC with any fuel injection scheme. Although, I'm not sure why you'd want a good ignition system going to waste sparking a dumb injection system...
Larry West has (had?) an excellent writeup of the DI/APC retrofit on his web page. It's not terribly complex.
