People are quick to blame software, but software is fairly consistent and adaptive. Hardware however can deteriorate and break down. 20-25lbs of boost sounds like WAAAYYY too much for a tiny t25 to be pushing out even in a quick spike, I don't think it would be programmed that way. If one of the three BPV lines is disconnected, it can allow your boost to spike up uncontrollably and most certainly cause the condition you described.
