In answer to your questions:
1) Yes, dropping the pan to inspect for sludge and plugged oil pick-up screen, and removing the valve cover to inspect the cam galleys for sludge, will result in a very good idea of how clean the engine is.
2) Maybe 2006 and later. There are many causes of this sludge issue (the biggest being poor maintenance) but the PCV system also contributed. The latest version of the PCV system was installed at the factory on 2006 and later, I believe.
3) Frequent and sensible oil changes using a quality synthetic oil will prevent sludge too. I change oil at 5,000 mile intervals and have only used synthetic since I bought my car new.
