it sounds like you overextended the slave as you were taking the spacer out--- this can easily happen if the person working the clutch pumped it one too many times... I think you have to have the person just depress the clutch once and hold it to the floor, while you twist and pull to get the spacer out.
as others suggest, you can use the nut trick to remove everything, put the slave back together (check o-rings for damage), then start over. (the slave cylinder can get cocked to one side when overextended, just carefully diassemble and then re-assemble).
