Daunting read. You've thrown everything but the kitchen sink at it electronically and fuel-wise. Impressive! From the beginning, it has run good cold so seems all that stuff wasn't necessary. I'm probably being simplistic and equally wrong but I think it has to do with running lean, not too rich. If it was running that rich to be tough to drive, you'd be smelling gas out the exhaust and your plugs would be wet/black with soot. Assuming your timing is correct, I think you've got a plumbing problem letting unmetered air into the engine causing a lean condition when the thermal sensor shuts off the excess cold start fuel. I'd pressurize the whole intake system with a shop vac and look/listen for leaks in all the hoses, pipes, connections, intake manifold and vacuum lines. A squirt bottle with soapy water will help. Chances are, your problem is simple. Maybe even a head slapper. My 2c...
