I wound up pulling the steering column out, which is easy. The hard part is getting it back in!
I used a bit of cut up rubber band to hold the washers in place and a bit of 2x4 to support it while I got the bolts in.
The "easy" way to do this would be to just remove the whole shaft by undoing the UJ at the bottom of the column and very slightly spreading the lower joint opening with a screwdriver in the clamp holding it in a vice.
Sadly it looks like the rebuilder (Phoenix Rack and Axle) managed to put the input shaft in 180 degrees off with the rack centered - so it has to come out again! Annoying. I did not know this was even possible. They did have trouble rebuilding it and had to resleeve it so it would not leak, which is probably a good reason not to DIY the job.
I guess not all vehicles have the bolt notch in the shaft, so it usually does not matter!
