I look at yaofeng's comment this way - These are the common reasons for this issue, and should be checked. I don't think it's a matter of insulting your mechanic-hood.

I work in an industry (jet engines) with very complex machines and very, very experienced workers. And still, stuff happens. We try and engineer out the possibility of errors - it's called mistake-proofing - because even the best mechanic can make a mistake. And most car engines, especially around the head, aren't mistake-proofed by a long shot.

I consider myself pretty experienced and ultra-careful (that aerospace training), and I have, with great concentration and careful thought, put something in upside down, or left it out entirely.

The first rule of troubleshooting - don't believe everything you know - basically, don't assume. Don't assume a new part won't fail right away; in fact, right away is the most likely time for a component to fail. Don't assume you couldn't have made a mistake assembling something - "Maintenance Induced Failures" are a significant portion of all faults, even with highly-trained, FAA certified mechanics with work reviewed by inspectors.

The best way to approach a troubleshooting problem is to assume nothing - take a methodical, step-by-step approach to checking everything. How would you approach the problem if somebody handed you this car and you had no history on it? That's the best way. If you know it isn't X, and you don't check X, there's a good chance it is X.

It's not personal. Gremlins sneak in and mess stuff up when your back is turned, or crease that headgasket as the bolts torque down.

If somebody handed me this car with no history, based on a few reported facts, I'd say head gasket or the rings either aren't seated right or aren't installed right. I'm pretty handy with cars, and I've had opportunity to kick my self more than a few times.
