Mine do have caps that cover the lug bolts which is the problem. The cap does not sit all the way down in as it is hitting the tall Saab bolt heads. A smooth security nut or a standard acorn would clear.
I don't understand what your saying in the first paragraph except that I agree that the little stud is there to aide in mounting. I need to convert to a stud system to get more clearance than the stock Saab lug bolts allow. The other option is to cut the head down slightly. Not an easy thing.
