When you finish a head gasket replacement, and can't find the exhaust manifold washer for the stud between Cyl 2&3. You assume it ended up in the oil catchment, or perhaps in a seem in the garage floor. You assemble the car, and find that the washer is the thing preventing your turbo from spinning the moment it creates enough vacuum to pull the washer into a tight spot between turbo fan blade and housing ;o)
It was only a disassembled turbo, and some pliers (two people involved here) to extricate the washer from the turbo. With only a couple of (very small) nicks in the turbo vanes, the episode was over. That was 30,000 miles ago. No trouble since!
Note: this is not something I'm proud of.
