A lot of this, on pickups, is because they have a lot of weight in the back. In the winter, people have sand, firewood, logs, etc for traction, or they might have a snowmobile in back.
In the summer, they have an ATV or riding mower weighing it down.
If it's an out of warranty SUV or truck, probably the leaf springs are sagging because the vehicle was used for it's intended purpose and is suffering as a result.
A weighed down or sagging truck/SUV will aim it's light too high.
Inspection stations (in Maine) are supposed to check for headlight aim, but nobody does. Inspections are annual and inexpensive for the amount of time they take and disruption they cause. This means an inspector will either ignore some of the more difficult things that are to be checked, or be way too thorough and use the inspection as a means of convincing you to spend lots of money on repairs at their shop before you can drive again.
