I did this job earlier this year on a '98 V70, which should be materially similar.
You can pull the transmission out the wheel well, but I have done that job on other cars and it always sucks. I took mine out the top attached to the motor. I figured doing so would give me an opportunity to do some stuff to the motor on the stand rather than squish myself into the engine bay.
It took me about three hours to pull the thing, and maybe slightly less time to put it back. Overall, it was pretty painless - not difficult work by any stretch of the imagination, just a fair number of details. Honestly, the most annoying, time consuming part was getting the driver's side axle out of the transmission.
There is a walkthrough on Matthew's Volvo Site for this job on an 850, and it's materially similar on the S/V70s. Frankly, having never touched any of these cars before the walkthrough wasn't all that helpful - it's all pretty obvious. Definitely pick up a Haynes manual (the Chilton one sucks) just so you have all the torque specs. Lots of TTY bolts and whatnot, so having the right specs is important.
Even if you do nothing other than replace the transmission in terms of preventative maintenance, I'd highly encourage you to replace the torque converter seal, rear main seal, and both axle seals. It will never be easier. I also replaced all the cooling hoses, vacuum hoses, motor mounts, a bunch of gaskets and seals, etc. Also a good time to replace the timing belt and water pump if the car is due (mine wasn't even close).
Hard to say whether it's worth it financially or not. The '99-'01 ('02?) cars have some expensive systems that are prone to failure like the electronic throttle body. That's why I went for a '98 - it's just an 850 in V70 clothing. I'd see what transmissions go for and see where that gets you. I bought my used transmission for $250 with a warranty, which made the $750 Volvo pretty darned appealing. Not sure what the later 5-speeds go for, but I was told by the dismantler that the 4-speeds rarely fail and so they sell them cheap, whereas the 5-speeds seem to fail more often and they charge more. YMMV.
FWIW, I ended up putting about $1800 worth of parts into the car, so now I have a $3000 '98 V70. Thing runs like a champ - did 400 miles in it today back and forth across central California in 100 degree weather. It is NOT fun to drive. People hate on the 9000 because of it's un-sharp handling but the Volvo is a whole different world of sedate-ness. It's a comfy highway cruiser but not a sporty ride. It is quick in a straight line, however, which is hilarious fun for me and awfully surprising to other traffic. ;)
