Some engine configurations produce more vibration than others due to various geometries. Consider a BMW opposed twin motorcycle engine, both pistons go out at the same time and come in at the same time, this is very well balanced and vibrates very little. Contrariwise a Harley V-twin vibrates a lot due to crankshaft/piston assembly weight imbalance and oddball uneven firing intervals. V8s have twice as many firing impulses per revolution as 4-cylinder engines so tend to be smoother. V4s (and inline 4s) are not naturally balanced and one way to counteract the vibration is to have a small shaft with counterweights that smooth out the vibration. Some inline 4s have two balance shafts. On the V4 the balance shaft rotates at crankshaft speed but in the opposite direction, CCW, and it is the balance shaft that extends through the front cover with a pulley to drive water pump, fan, and alternator. When the balance shaft gear breaks the engine runs fine but you suddenly have no cooling or charging, so it won't run fine for long.
