I have always changed the belt and all pulleys at 60K mile intervals, as recommended by Saab. And, I have never had a failure following that preventive maintenance schedule.
However, I did try to go 120K miles on my 2000 9-5; a pulley disintegrated at 108K miles and I had to be towed to a dealer 70 miles away (thanks to AAA).
Now, I change ALL pulleys and the belt at 60K mile intervals on my 9-5 and my wife's.
