I always remove the 12mm plug first and the spring. It is also a little plastic peg inside the spring in the bottom.
This way I can use a shallower 27mm socket instead of deep. Also, it keeps the last setting of the tensioner so I can study it right away and see how far out it is.
Then I install the tensioner first and insert the spring with peg afterwards. It is a little bit tricky to push and thread the plug back on so all depending on dexterity of your fingers and multitasking it might be more or less difficult.
