Exhaust manifold stud loose

I had the manifold gasket replaced some time ago by a Saab indy. I noticed that the lower stud closest to the battery was loose. It's also missing a sleeve which I see on other 9-5s. EPC and WIS don't say where the shorter sleeveless and longer sleeved studs go. Are the longer ones the outermost four with the three center ones being shorter? I also see parts places selling what seem to be kits of seven equal length studs. The ones on this car use a 12mm nut if that tells anything.
For now, I removed the stud, cleaned it, put light anti-seize coating, and re-torqued the stud. Do I need to do more? Should I be checking/re-torqueing periodically? How do I access the hidden ones? Remove the heat shield? What's the purpose of the sleeves other than the upper ones which hold the shield? Is it that a longer stud has more elasticity and thus a lower reduction in clamping load when it heats up? That might explain why the short one has come loose. If so, should I swap some short ones for long ones?
