regardless of which one's been replaced in the past, the replacements certainly can fail after a few years .. especially if aftermarket parts were used.

It's unlikely that the rear mount is causing the problem, especially given that you said they just replaced the upper passenger side mount (i.e. the one over/near the belts), and then the vibration started when you got the car back (or did I misunderstand?). I would investigate the mount they just replaced when doing the head gasket... it's possible that they didn't torque it down properly, which could cause the vibration.

I would focus on that mount, and then if it's ok.. check the one under the battery, for the transmission side.. as those are almost always the culprits..
