if you do a search you will find that many people have the pedal hole elongate as JohnK mentioned. Maybe yours got so sloppy that the pin slipped out even with the cotter pin on, or the pin and/or cotter pin broke. (Anything's possible seeing how you've got 317K miles). So take a good look at that hole, you can do as JohnK says and oversize the hole and pin, but then you have to oversize the master hole too. Many people weld up the hole and redrill to maintain the original size, if you don't have a welder, go to a shop, it can't be real pricey for a 5 min. weld job. Oh and it is a bit tight to reach under there with the seat in place, but it can be done and I myself am way to lazy to pull the seat for this fairly simple job.
