Running voltage should be around 14 volts, say 13.5-14.5 volts without much stuff running(clean grounds and no lights, fan, rear window def,etc). If you're down close to 13.0 volts, I'd say you're on the low side. With added electrical load, voltage will drop, but should stay above 12.6. After you've run the car for awhile and then shut it off, battery voltage should be 12.58 (approx 12.6).
If your running voltage is up around 14, but engine off battery voltage is below 12.58 and/or drops steadily with minimal load, your battery may have reached the end of its useful life. They only have so many complete discharges before they give up. The battery dealer can tell in a minute with a load tester that basically checks how well the battery can withstand a load simulation and then recover to the 12.58 volts.
I have never had a regulator go bad, but I think you can get voltages that are too high. Seems like brushes last somewhat more than 100k miles. Check yours just to be sure. I think if they are less than 5mm, time to replace. The first time I soldered in new brushes, and it's a job that requires 4 arms. After Jim Moncrief of Sobstory laughed at me, I have since just replaced the whole regulator...less than $30.
