The plastic is liekly to keep it in shape for shipping (??).
Turn the boot inside-out, place small end over reverse knob (yes- tight fit), tie off (OEM is elastic string, my wifes Godiva chocolate box wraps work just fine).
Then pull the large end back over (right-side out) itself to base. Attach three corners and use a plastic or wood "something" to strech the final corner over the metal bracket.
If you are still unsure- call a friend with 5 speed over for a beer :)
