Lift up the reverse collar and see if you can move the shifter a little bit to the right and get it into 5th gear. The way the shifter assembly is set up, there is a ridge in the plastic cover at the bottom of the assembly that gives the shifter something to line up with to push it into 5th. When you lift up the collar, it lifts up a pin that lets you bypass the ridge. It's worth a try.
If that doesn't work, then there's something internal to the transmission that's amiss. IIRC, there is an internal clip or pin that comes off in rare circumstances and causes problems getting into 5th. I seem to remember a couple of threads about it in the past couple of years.
