I have a portable unit in my Saab and a head unit in my other car.
Go for the sat-ready head unit.
There is no difference between the sound quality (the portable feeds the aux-in on my radio) but there is a huge difference in ease-of-use. The head unit is easier to control, easier to manage, and easier to see what's playing.
As fas as Sirius vs XM, my cars have Sirius and my wife has XM. They play pretty much the same music, but only one has MLB, one has NASCAR, one has Howard Stern. XM does have a better traffic/weather system (one city per channel) where Sirius splits the traffic/weather for two cities between one channel (ie - Chicago/St. Louis is on one channel, you may have to wait 4 minutes to hear what you need).
