I would first check the compression and see if it is across the board mostly the same. If it varies as I mentioned before, then you have an imbalance in the combustion causing the vibration, but that doesn't mean there isn't another problem.
If the compression is equal across the cylinders, and the fan belt removal is inconclusive, then I would suspect the flywheel, pressure plate, clutch assembly.
Or, come to think of it, if the head has ever been off, then you could be a tooth off on the cam timing and that causes a weird thumpy idle vibration.
