I've only done this once and didn't have the tool.
I put the shifter into third, loosened the linkage, then made sure the shift rod is fully engaged in third in the transmission, aligned the hole and stuck the appropriately sized drill bit in there then tightened the linkage.
Only difference between what Steven is saying is that once I loosened the linkage I then made sure that third was fully engaged on the transmission.
It's easier if you have a helper to be sure the positioning doesn't change which I would think is what the tool is for.
