I had the exact same problems several years ago, very intermittant, until one day when it died and refused to start. I banged on the ignition amp, and got the car started (highly scientific!). I replaced the ignition amp, only to have the same problem reappear a year later. This time, replacing the amplifier module didn't do squat. However, replacing the distributor with a good used one, has rid my car of the problem for over four years now.
The only other cause would be the ignition coil, but it's my understanding that not only do they rarely fail (still on the original in my car), but they don't produce symptoms like we have been describing.
