I've only had this happen once. At the time, I didn't have my normal tensioner tool and was using about 6 inches of 700lb chain and hooking one link on the top of the tensioner, compress the tensioner, then hooking a link on the bottom and letting the chain keep it compressed. I got the chain on the tensioner but when compressing to remove it i couldn't get it off so I pushed back too hard and stripped the bolt. The thing only compresses so much. Maybe he's not using the proper compression method and pushing on the bolt too hard, this could also account for tensioner failure due to compressing it too much and too quickly.
Btw, it's $12ish from the dealer I think
Anyway, ask him about that and what method he uses to keep it compressed. I learned my lesson the hard way.
