You have 2 problems and one recomendation.
First, by the sounds the fan is making the bearings in it are toast. It's time for a new blower motor.
Second, the thermistor (thermal fuse) on the fan speed controlling resistor has opened up causing the fan to only work on the 4 setting. It's time to replace the fan speed resistor as well, but I would change out the blower motor before this is replaced. It would also be a good idea to check the fan speed selector switch, and wiring to the resistor for dead shorts. Something caused the thermistor to get too hot and open up.
I have to check this out in my own 92 9000S, it likes to eat these when the fan speed is set below 2 or switched to 0 quickly.
And third, since the blower motor is on it's way out I'd recomend replacing the heater core at the same time, since you have to take the blower motor out to get to the heater core.
If you need the wiring schematic for this let me know. I can scan it and save it as a .pdf file.
