A few things to check. First, did you cross thread the bolt that goes through the center of the bushing? That would prevent you from tightening the bolt. Second, try putting the bolt through just the bushing center with the dog-bone out of the car. Is there too much clearance? Third, how sound is the mounting of the bracket to the shock tower? The holes that mount the bracket have been known to strip or rust out, which makes the whole thing loose.
