I'm pretty sure the tensioner pulley can go in either way - I just grabbed one, and the distance from the plane of the pulley to the insert is the same on both sides.
Is the belt hanging over the edge of the pulley? The belt should be centered, or pretty darn close. If it were close to the edge on one side, or worse, over the edge on one side, I'd expect it will walk off. It does help to turn the engine over a couple of times to let things straighten out.
First, I'd make sure the tensioner assembly is bolted properly to the engine. I've seen one of the bolts go, and the tensioner is now cocked. The bolt could be broken, but still in place.
The primary reason I've seen for the belt in the wrong place is a bad harmonic balancer (main pulley). I've seen the main pulley start to fail, and walk out a little - this offsets the tensioner. I'd do the chalk test on the main pulley - or at least give it a good look to see if the rubber sandwich was failing.
