Balance Chain Question

Someone help me understand this. I'm looking at my balance chain and understand the alignment of the intake balance shaft. It's aligned as it's suppose to be. My question is on the exhaust balance shaft. It wasn't aligned properly, but it doesn't appear to be driven by the chain. I can freely spin it. The shaft above it (the idler pulley) is what has the teeth on it. How does the exhaust balance shaft work? Seems to me that it'll be aligned when I put it back together, but as soon as I start the car, it could go to whatever position it desires.
