1) use a small vise-grip plier to pull on the cable itself. Works great.
2) lay down in the footwell area with a flashlight and look at what happened. The end of the cable is a bent piece of metal, that passes through a hole in the release handle, and is bent back around. Over time, and use, the bend opens up, and eventually, the metal pulls back through the hole. Feed the metal from the cable through the hole again, and using a pair of pliers, or two, bend it around, and pinch a loop shut in the cable, around the hole in the handle.
